Accident Summary:
THE ACCIDENT OCCURRED DURING THE CONDUCT OF A TRAINING MISSION ASSOCIATED WITH THE 101ST AIRBORNE DIVISION'S OH-6A TRANSITION SCHOOL. AS STATED IN THE STATEMENTS OF THE PERSONNEL INVOLVED, THE FLYING PERIOD BEGAN AT CAMP EAGLE, RVN, WITH AN IP BRIEFING AND TAKE OFF AT APPROXIMATELY 1300 HOURS. CW2 COWAN'S FIRST STUDENT WAS GIVEN 1:30 OF FLIGHT INSTRUCTION AND A CHANCE TO FAMILIARIZE HIMSELF WITH HIMSELF WITH THE AIRCRAFT. DUE TO UNFAVORABLE WIND CONDITIONS AT CAMP SALLY, NO ATTEMPT WAS MADE TO DEMONSTRATE OR PRACTICE AUTOROTATIONS AND THE UNEVENTFUL INSTRUCTIONAL PERIOD TERMINATED AT 1430 HOURS. THE AIRCRAFT WAS REFUELED AND CW2 COWAN CHANGED STUDENTS TO BEGIN THE SECOND PERIOD OF INSTRUCTION. THE SECOND PERIOD OF FLIGHT INSTRUCTION CLOSELY RESEMBLED THE FIRST EXCEPT FOR A FAVORABLE WIND SHIFT ALLOWING A SAFER DEMONSTRATION AND PRACTICE OF BASIC AUTOROTATIONS. CW2 COWAN DEMONSTRATED THREE BASIC STRAIGHT IN AUTOROTATIONS AND ASKED WO1 KELLY IF HE WOULD CARE TO ATTEMPT THE MANEUVER. WO1 KELLY ENTERED THE MANEUVER IN THE APPROPRIATE MANNER AT THE PRESCRIBED ALTITUDE AND PROCEEDED TO EXECUTE WHAT APPEARED TO BE A NORMAL EXECUTION OF THE AUTOROTATION. AT APPROXIMATELY 15 FEET OF ALTITUDE, INITIAL PITCH WAS APPLIED IN AN ATTEMPT TO DECREASE THE RATE OF DESCENT AND AGAIN APPLIED COLLECTIVE PITCH AT 8 TO 10 FEET OF ALTITUDE. THE AIRCRAFT FELL FROM THIS ALTITUDE AT AN EXCESSIVE RATE AND LANDED HARD ON THE RUNWAY. UPON IMPACT THE TAIL BOOM SEVERED FROM THE FUSELAGE OF THE AIRCRAFT AS A RESULT OF THE MAIN ROTOR BLADES STRIKING THE TAIL SECTION. CW2 COWAN WAS UNAWARE OF HIS TAIL BOOM LOSS AND ATTEMPTED TO FLY THE HELICOPTER OFF THE RUNWAY. UPON APPLYING POWER AND INCREASING COLLECTIVE PTICH FOR HOVERING FLIGHT, THE AIRCRAFT SPUN VIOLENTLY TO THE RIGHT AND AN IMMEDIATE HOVERING AUTOROTATION WAS INITIATED. THE AIRCRAFT CAME TO REST APPROXIMATELY 45 TO THE RIGHT OF ITS LANDING DIRECTION.\\

PILOT WAS FLYING VR AROUND THE PERIMETER WHEN HE WAS TOLD TO GO AND CHECK OUT A LOCATION ONE MILE SOUTH EAST OF THE PERIMETER. HE WAS IN LEVEL FLIGHT AND HAD HIS LANDING LIGHT ON. HE WAS LOOKING OUT RIGHT DOOR AND WHEN HE LOOKED FORWARD HE SAW A TREE IN FRONT OF HIM. HE PULLED PITCH AND REAR CYCLIC AND THEN THE AIRCRAFT SKIMMED THROUGH THE TOP OF THE TREE. PILOT THEN BROUGHT THE AIRCRAFT BACK TO PHU BAI AND SHUT DOWN.\\
